After the patient is discharged from the hospital, our dedicated primary care team conducts thorough and timely follow-up examinations to ensure that the best medical treatment is provided to the patient. All medications taken by or recommended to the patient are reviewed.
A confirmation for the same is obtained from the hospital. If the patient requires a follow-up examination after being discharged, our team gets in touch with the relevant specialties and ensures that a smooth transition takes place. We do not leave any scope for ambiguity that could potentially create gaps in delivering care and treatment to the patient.
